If you’re pregnant or postpartum and considering pelvic physical therapy, you probably have questions — and that’s completely normal. Whether you’re dealing with back pain, bladder leaks, pelvic pressure, or wondering how to safely return to exercise, understanding what to expect can make the process feel far more comfortable and empowering.
At Fundamental Physical Therapy, my goal is to help you feel informed, supported, and confident as you begin your pelvic health journey. Below are some of the most common questions I hear from expectant and new moms considering pelvic floor physical therapy during pregnancy and postpartum.
How Often Will I Need Pelvic Physical Therapy?
The frequency of pelvic physical therapy visits depends entirely on your individual symptoms, stage of pregnancy or postpartum recovery, and personal goals.
During Pregnancy
How often we work together during pregnancy depends on:
Whether you’re experiencing symptoms (pain, leakage, pelvic pressure, etc.)
How far along you are
Your activity level and birth goals
Most expectant moms benefit from:
One visit to learn how to manage pregnancy-related body changes
One visit focused on labor and delivery preparation
One visit to plan for the early postpartum period
During Postpartum (the “Fourth Trimester”)
Most postpartum patients are seen for 2–3 visits in the early weeks after birth, once cleared by their provider, and then we adjust based on symptoms and goals.
Spacing visits allows you to:
Practice new exercises gradually
Build confidence with movement and recovery
Avoid feeling overwhelmed
As symptoms improve, visits may space out to every other week or monthly until you’re ready to maintain progress independently.
Every treatment plan is personalized. The goal is never long-term dependence on therapy — it’s to help you feel strong, capable, and confident in your body.
Can Pelvic Physical Therapy Help Prepare Me for Labor and Delivery?
Yes! Pelvic floor physical therapy during pregnancy can play a valuable role in preparing your body for labor and delivery.
Pelvic PTs help with:
Pelvic floor awareness and coordination
Breathing and pressure management
Labor positioning strategies
Reducing fear and tension around birth
Understanding how to relax and coordinate your pelvic floor can be just as important as strengthening it — especially when preparing for delivery.

Will There Be a Pelvic Exam?
A pelvic exam is always optional and is a decision we make together.
During Pregnancy
In a well-established, low-risk pregnancy, an internal pelvic exam may be offered to assess pelvic floor muscle function if symptoms like pain, bladder or bowel issues, or concerns about delivery are present.
During Postpartum
Once you’ve been cleared by your physician (typically at the 6-week follow-up), an internal pelvic exam can help assess:
Muscle strength and coordination
Healing after vaginal or Cesarean birth
Contributors to pain, weakness, or leakage
That said, you are always in control of your care. You can defer or decline any part of the exam or treatment at any time. There are many effective ways to assess pelvic floor function without an internal exam.
Your comfort, privacy, and consent are my top priorities. Any exam performed is gentle, fully explained, and done with care and respect.
If I Didn’t Have a Vaginal Delivery, Do I Still Need Postpartum Pelvic Floor Therapy?
Absolutely. A common misconception is that pelvic floor issues only affect women who have vaginal deliveries. In reality, pregnancy itself places significant strain on the pelvic floor, regardless of delivery method.
Even after a C-section:
Your pelvic floor supported months of increased load and pressure
Posture and breathing patterns changed throughout pregnancy
Abdominal surgery affects core coordination and creates scar tissue
Pelvic Physical Therapy After a C-Section Can Help With:
Scar tissue mobility to reduce pulling, numbness, or discomfort
Restoring core and pelvic floor coordination
Back or hip pain related to post-surgical movement changes
Preventing long-term issues like leakage, pelvic heaviness, or pain
Your pelvic floor and core function as a team. Supporting one means supporting the other.
I’m Not Having Pain or Leakage Postpartum — Do I Still Need Pelvic PT?
That’s wonderful news — and it doesn’t mean pelvic PT wouldn’t be helpful.
Pregnancy and delivery (of any type) create hormonal and mechanical changes that can quietly affect:
Muscle strength
Joint stability
Posture and balance
Pressure management during movement
These changes may not cause symptoms right away but can contribute to issues months or even years later.
A postpartum pelvic floor checkup is an opportunity to:
Assess pelvic floor and core function proactively
Screen for diastasis recti or movement imbalances
Get guidance on safe return to exercise, lifting, or intimacy
Prevent long-term dysfunction through early education
Think of it as your postpartum checkup — but for your muscles.

Can I Bring My Children to Appointments?
I love meeting families, but if possible, it’s best to attend your initial and follow-up sessions without children. This allows for:
Open conversation about personal topics
Focused, uninterrupted care
Time just for you
That said, I understand that childcare isn’t always predictable. If needed:
Infants who are content in carriers are welcome
Please keep your appointment — we’ll make it work
